Acknowledgment of Risk

I, the undersigned, acknowledge that participating in surf lessons involves inherent risks, including but not limited to:

• Physical exertion and potential injury from falls, collisions, or contact with surfboards, other participants, or natural elements

• Exposure to changing ocean conditions, including waves, tides, currents, and marine life

• Weather-related hazards such as sun exposure, dehydration, or lightning

I understand that while Brandon Sassen and fivepoint9ine designs will take reasonable precautions, they cannot guarantee my safety.

Medical Disclosure

I confirm that I am physically fit to participate in surf lessons and have disclosed any relevant medical conditions, allergies, or limitations to the instructor.

I grant permission to receive medical treatment if deemed necessary.

Release of Liability

In consideration of being allowed to participate in surf lessons, I hereby release and hold harmless Brandon Sassen, fivepoint9ine designs, and any affiliated staff or volunteers from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in surf lessons.

Photo & Media Release

I grant permission for fivepoint9ine designs to use photographs or video footage taken during lessons for promotional or educational purposes. Any photos or videos taken can be sent to you after your lesson if an email address or phone number is provided.

The lessons were fantastic! I broke a board and they helped me repair it, while teaching me new skills. Truly enriching my surfing experience!
